Lost Driving Licences, Counterparts or Tags
An application for the issue of a reprint of the driving licence may be made with the Authority, if a valid driving licence has been lost, stolen or damaged. The replacement driving licence shall have the same effect, including the expiry date, as the original driving licence. To issue a reprint of licence, the licence holder has to fill in a declaration form VEH 39 - English or VEH 39 - Maltese stating the reason why the licence is being reissued and submit it showing proof of identification. The new licence photocard is issued against a fee of 18.00.
An application for the issuance of a replacement of the licence counterpart may also be made, if a valid licence counterpart has been lost, stolen or damaged. The replacement licence counterpart shall have the same effect, including the expiry date, as the original licence counterpart. To issue a replacement of the counterpart, the licence holder has to fill in a declaration form VEH39 - English or VEH39 - Maltese stating the reason why the counterpart is being reissued and submit it showing proof of identification. The new licence counterpart is issued against a fee of 8.00.
An application for the issuance of a replacement of the Driver Permit Tag may also be made, if a valid tag has been lost, stolen or defaced. The replacement driver permit tag shall have the same effect, including the expiry date, as the original tag. To issue a replacement of the tag, the tag holder has to fill in a declaration form (VEH39) stating the reason why the counterpart is being reissued and and submit it showing proof of identification. The new Driver Permit Tag is issued against a fee of 20.00.

- Payment of 8.00 (to issue licence counterpart)
- Payment of 20.00 (to issue Drivers Permit Tag)
- Copy of the identity card
